Abstract
결론
저자 등은 1994년 1월부터 1996년 7월까지 조선대학교 부속 치과병원 구강악안면외과학교
실에 내원하여 급성 또는 만성의 과두걸림 증례 환자 8명을 대상으로 측두하악관절 상관절
강 세정술 후 주기적인 임상검사와 방사선사진을 통해 다음과 같은 결과를 얻었다.
1. 국소마취하에서 시행한 측두하악관절 상관절강세정술은 술식이 간단하며 합병증이 적
다.
2. 환자가 내원할 때까지 비정복성 관절원판의 전위에 의한 과두걸림의 기간은 평균 11.9
개월이었다.
3. 측두하악관절 상관절강 세정술 전 평균 개구량은 25.4mm, 술 후 개구량은 39.8mm이었
으며, 평균 추적 검사시간은 17.1개월이었다.
4. 측두하악관절 상관절강 세정술 전후의 주관적인 동통(0-15)평가시 8명의 환자 중 7명
의 환자(87.5%)에서 만족할만한 동통의 감소를 가져왔다.

The purpose of this study was to access the effect of TMJ arthrocentesis in locking 8
patients with anterior distal displacement without reduction ( 'closed lock' ).
TMJ Arthrocentesis was defined as needle puncture of the joint space and usually
combined with lavage. After lavage, the mandible is gently manipulated to evaluate the
movement. Arthrocentesis with irrigation has been used successfully with few
complications and low morbidity. Postoperative therapy requires repetitive opening and
excursive exercises to maintain and improve the function.
The mean follow-up period was 17.1 months(range, 8 to 30months). Preoperative and
postoperative mean maximal interincisal opening(MIO) was 25.4mm preoperatively and
39.8mm postoperatively. The pain was reduced in 7 out of 8(87.5%) patients with limited
mouth opening. The result of this study indicate that TMJ arthrocentesis is an effective
means of controlling pain and improving jaw movement in patients with closed lock.
